Remove App.ready

App.ready can cause an app to run on multiple threads.

If on thread A:


  app.run   # state == RUN
  app.stop  # state == STOP

Then on thread B, before run terminates:


  app.ready # state == READY
  app.run   # ok to go!

App already ensures it will reset ready regardless of how it exits.
